| dc.description.abstract | Introdução: O transplante renal é o tratamento de substituição da função renal de melhor custo-benefício. O objetivo deste estudo foi analisar a lista de espera para transplante renal de acordo com a reatividade contra painel calculada (cPRA) e seus desfechos em um hospital terciário. Resultados: A média de idade foi de 45,39 18,22 anos, sem diferença entre os grupos de cPRA (p=0,328). O sexo masculino foi o predominante (61,2%), porém a proporção foi diminuindo linearmente com o aumento do cPRA (p<0,001). A distribuição de pacientes conforme os painéis foram: 0% (n = 390), 1% - 49% (n = 517), 50% - 84% (n = 269) e ≥ 85% (n = 226). O transplante foi realizado em 85,5% da amostra com uma mediana de 8 meses (IC 95%: 6,9 - 9,1) em lista. Houve diferença significativa na comparação da curva de tempo de espera por cPRA (Log Rank = 188,0 p<0,001). O cPRA ≥ 85% teve uma mediana de tempo até o transplante de 36 meses (IC 95%: 28,1 – 43,9), significativamente maior que os demais grupos. O cPRA ≥85% associou-se significativamente com a proporção status de ativos e óbito em lista de espera (p=0,004). Nos pacientes submetidos ao transplante não observamos diferença na sobrevida do enxerto e do paciente entre os diferentes cPRAs. Conclusão: O tempo de espera até o transplante renal foi significativamente maior em pacientes com cPRA ≥ 85%, no entanto a sobrevida do enxerto e do paciente foi similar nos diferentes grupos de cPRA. | dc.description.abstract-en | Introduction: Kidney transplant is the most cost-effective renal replacement therapy. The aim of this study was to analyze the kidney waiting list according to cPRA and its outcomes in a tertiary hospital. Patients and methods: 1,640 patients who were on the waiting list between 2015 and 2019 were included. For the analysis, waiting times were compared according to panel reactivity. The presence of anti-HLA antibodies, considered the mean intensity fluorescence (MFI), was above 1000. Results: The mean age was 45.39± 18.22 years, with no difference between the cPRA groups (p=0.328). Male sex was predominant (61.2%), but the proportion decreased linearly with the increase in cPRA (p<0.001). The distribution of patients according to the panels were: 0% (n = 390), 1% - 49% (n= 517), 50% - 84% (n = 269) and ≥ 85% (n= 226). The transplant was achieved in 85.5% of the sample, within a median of 8 months (CI 95%: 6.9 - 9.1m). There was a significant difference comparing the waiting time curve by cPRA (Log Rank = 188.0 p<0.001). The cPRA ≥ 85% had a median time until transplantation of 36 months (CI 95%: 28.1 – 43.9), significantly higher than other groups. The cPRA from 50% - 84% and 1% - 49% were not different between them (7.0 months CI 95%: 5.2 - 8.8 and 6.0 months CI 95%: 4.8 – 7.2, respectively), but significantly higher than cPRA zero(5.0 months IC95%: 6.9 –9.1). The cPRA ≥85% was notably associated with proportion status of actives and death on the waiting list (p=0,004). In patients undergoing transplantation, it was not observed any difference in graft and patient survival among the different cPRAs. Conclusion: The waiting list time until kidney transplant was significantly longer in patients with cPRA ≥85%, however graft and patient survival was similar in the different cPRA groups. | |
